Customer intake
Customer intake is the "How did you find us?" question. It asks a new customer where they heard about you, and it reports the answers.
Go to Admin > CRM > Customer Intake.
License required
Customer intake needs the Intake Forms license add-on. Without this license, both switches below are locked.
The two places it asks
Intake has two switches, and each one covers a different moment. Turn on one, the other, or both.
Go to Admin > Settings > Features and find the Customer Intake group.

Purchase Intake is greyed out until billing is on.
Each switch has its own list of answers. Turning on registration intake does not surface the purchase answers, and the other way round.
TIP
The Customer Intake page stays in the CRM section even when you turn the rest of CRM off.
Skip the question on one product
A product can opt out of the checkout question. Open the product in Admin > Billing > Products, go to Display Options, and untick Show intake question during purchase. Read Products.
Build the answer list
The Options tab has two panels: Registration Sources and Purchase Sources. They work the same way.
- Go to Admin > CRM > Customer Intake.
- Open the Options tab.
- In the panel you want, pick a type: Option or Text Input.
- Type the label into the box.
- For a text input, add placeholder text. This is optional.
- Click Add.
A green banner reads Source added.
Empty state: No sources configured yet. Add one below.
The two types

A Text Input row carries a teal Input chip in the list, so you can tell the two apart.
WARNING
Text inputs work on the registration form only. The checkout renders the dropdown and nothing else, even when you add a text input to Purchase Sources.
Reorder, rename, and switch off
- Click the up and down arrows to change the order. The order saves at once.
- Click the label to rename it. Press Enter to save, or Escape to cancel.
- Use the switch on the right to take a source off the customer form. An inactive row is dimmed.
- Click the bin to delete a source.
WARNING
The bin deletes a source at once. The panel does not ask you to confirm.
A source that customers already picked is not really deleted. The panel makes it inactive instead, so your old reports keep working. It comes back into the list greyed out.
You can rename a source at any time. You cannot change its type after you create it.
What the customer sees
On the signup form
A dropdown sits under the password fields, reading How did you find us?
The first entry is -- Skip --. Then come your active options, in the order you set, and Other (please specify) last.
If they pick Other (please specify), a text box appears with the prompt Please tell us how you found us.
Each active text input then follows as its own field.
The dropdown is hidden when you have no options in the list.
At checkout
A dropdown labelled How did you find this product? It works the same way, and the Other prompt reads Please tell us how you found this product.
TIP
Every intake question is optional. The dropdown always offers -- Skip --.
Read the answers
One customer at a time
Open a customer at Admin > CRM > Customers. The Account Details card has a Referral Source field with what they picked. A customer who skipped the question reads N/A.
WARNING
The profile shows the dropdown answer only. It does not show what a customer typed into an Other box or into a text input. Those answers live in the Analytics tab, and they are not tied back to a name there.
All the answers together
Open the Analytics tab on the Customer Intake page.
Set Context to Registration or Purchase. Set a From date and a To date to narrow the range. The To date is included. Click Clear dates to go back to everything.
Four cards run across the top:

Responses by Source lists each source with its count and its share, and a bar. Free-text answers appear at the bottom as one italic row, Other (free text).
"Other" Responses lists the distinct answers that people typed. This panel appears only when there are some. It shows up to 50 different answers.
Empty state: No responses in this range.
